Interactive SWUP Application Form

Each SWUP application form is an interactive PDF which lets you fill up the form and submit it via email. Filling up the form is simple and easy.

Instructions:

  1. If your computer doesn’t have Adobe Reader installed, download a free copy here.
  2. Download the appropriate form to your computer:

  3. Fill up the text fields by clicking on the areas you want to fill up and typing in the appropriate information.
  4. A checkbox can be ticked by clicking on it.
  5. After filling up the form, you can submit it in one of two ways:
    1. Click on the Submit button. An email with your filled up form attached will open in your computer’s email program.
    2. Save the filled up form and submit it to pldtswup@pldt.com.ph.
